Request Duplicate License
Submit a written request for a duplicate license to Agent Services Division, 1204 Mail Service Center Raleigh, NC 27699-1204. The request must include the premium finance company name, TAX IDENTIFICATION NUMBER, current mailing address and type of license.
License Name Change
Send written notification to Agent Services Division, 1204 Mail Service Center Raleigh, NC 27699-1204. The notification must include the previous and new name and tax identification number.
Other Business Changes For changes in license information including company ownership and financial data, submit change information to Agent Services Division, Attn: Accounting Section, 1204 Mail Service Center Raleigh, NC 27699-1204
Change My Address of Record
According to N.C.G.S. 58-2-69(b): “Every applicant for a license shall inform the Commissioner of the applicant's residential address and provide the applicant's e mail address to which the Commissioner can send electronic notifications and other messages. Every licensee shall give written notification to the Commissioner of any change of the licensee's residential or e mail address within 10 business days after the licensee moves into the licensee's new residence or obtains a different e mail address.”
Licensees change their company address by completing the Change of Address Form. Licensees are also required to provide current email addresses. Mail completed form to Agent Services Division, 1204 Mail Service Center Raleigh, NC 27699-1204; or fax to 919-715-3794; or send via e-mail to ASDfinancial@ncdoi.gov.
Agent Services Division only updates company addresses for Collection Agencies, Premium Finance Companies, and Motor Club Companies. Insurance companies should contact the Financial Evaluations Division to request an address change.
License Cancellation Requests
Return the license(s) with a written request to cancel them. Be sure the request is signed. If you are unable to find your license(s), submit a notarized statement requesting license cancellation. The statement must be signed and must indicate which licenses you wish canceled. Indicate THE PREMIUM FINANCE COMPANY NAME AND TAX IDENTIFICATION NUMBER.
Revisions to Forms and Marketing Materials
When the Department issued a premium finance company permit, specific forms, marketing materials, etc were approved for business to use in North Carolina. Approval by the Commissioner for revisions to these forms/materials must be obtained before the revised forms are used. The premium finance company should submit the forms/materials with a letter giving the date of NCDOI approval of forms/materials currently in use. Attach the current approved forms/materials and the proposed revision. In the cover letter, specify by paragraph and sentence # the proposed changes.
For example: Change sentence 2 in paragraph 1 from “xxx” to “xxx”.
