1. Estimate the percentage of how often spray polyurethane insulation is used in your jurisdiction for new residential construction? How often for retrofits?

2. Are permits required for retrofit installations in your jurisdiction? At what threshold is a permit required?

3. What documentation does your jurisdiction require on the spray foam products?

4. Have you encountered any difficulties in obtaining information from spray polyurethane foam manufacturers/vendors or installers?

5. Rate the quality and detail of the information provided by the spray polyurethane foam manufacturer.

6. Does your jurisdiction require full SPFI documentation for each project, or, if the product has been approved by your jurisdiction in the past, does your jurisdiction maintain a folder with the project information for future projects?

7. Under what circumstances would your jurisdiction allow intumescent coatings as an Alternate Methods and Materials to prescriptive thermal barriers and ignition barriers?

8. Does your jurisdiction require independent third party inspection of spray polyurethane foam insulation (SPFI) installations? If so, what information do you require in the inspection report?

9. Does your jurisdiction require independent third party testing of intumescent coating applications on SPFI? If so, what credentials do you require for the third party agency? If so, what information do you require in the test report?

10. What, if any, violations are typically noted in the spray polyurethane foam insulation (SPFI) installation by your staff or a third party inspection agency?

11. What, if any, violations are typically noted in the application of intumescent coatings applied to SPFI by your staff or a third party testing agency?

12. The Engineering Division has been informed of instances where SPFI applicators intentionally omitted the intumescent coatings from their initial bid where the coating was required as an alternate to a prescribed thermal or ignition barrier in an effort to gain an advantage in obtaining the job. Are you aware of this type of situation taking place in your jurisdiction? If so, please describe the circumstances. How often has this occurred?
