IN LINE OF DUTY DEATHS (LODD)
 
To Report a Fatality:
 
In the event of a fatality in your department/squad, immediately call the State Fire Marshal's Office at 1-800-634-7854 (this is a 24-hour number). You will be greeted by a voice menu, press “0” for the operator and ask to speak to someone about reporting a LODD. If you call after office hours (8:00 a.m. to 5:00 p.m.), you will be given this information to contact (via cell phone numbers); Tim Bradley (919)218-5311, Steve Sloan (336) 813-0088 or Larry Hughes (919)218-1319; a representative from the State Fire Marshal's Office will call you as quickly as possible.
 
A response team consisting of two OSFM personnel can be at your location within 24 hours.
 
Please have the following information ready when calling:
  1. Your name, department, a telephone number and/or cellular telephone number where you can be contacted.

  2. Time, date, and location of the fatal incident, as well as a brief description of the type of incident (Structure fire, vehicle accident, heart attack etc.).
