If you have identified a filing of interest from the federal Healthcare.gov "Rate Review" site, you can find some of the requested information noted above in the following fields:

Providing Comments via PPACA Filing Access

You may also make comments, which will be directly associated with the filing in question, by using the Department’s PPACA Filing Access: .

From that link you will be able to search from the landing page on the Company Name, the HIOS Submission Tracking Number for Rate Filing, SERFF Submission Tracking Number, Insurance Product Name, Effective Date range, and Market Type.
